Your first visit will usually include a cleaning, x-rays, periodontal evaluation, new patient exam, and a discussion of your oral health, including any needs and treatment options. If you have a complicated medical history, a cleaning may not be able to be performed the first day that you visit. In order to make your first visit seamless, please gather and bring the following to your first appointment:
- List of all medications taken including dosage, frequency and condition treated.
- All x-rays taken by your previous dentist (see Records Transfer section below).
- All necessary paperwork including your dental insurance card, drivers license, or another form of government issued identification.
Please alert us ahead of time if you have any of the following in your medical history: artificial joints, artificial heart valves, history of taking osteoporosis medication, heart attack, heart surgery, or stroke. Please also alert us if you are taking any heart medications or anticoagulants.
Records Transfer- Records are normally sent electronically. A few days before your initial appointment, please print and complete our Request for Transfer of Records form. Once that form is filled out, send it to your previous dental office so they will release all pertinent dental records to us.
